All forms, including for distributions, withdrawals, loans, and rollovers, can be found in My Documents. Follow the below instructions to download them.
Where do I find my 401(k) plan forms?
After logging in, click My Documents in the top menu.
Scroll down to Plan Forms.
Click the blue PDF icon next to the form you want to download it.
